Program for Out-of-Town Patients

At the Pelvic Pain Regional Specialty Center, we get phone calls every day from women across the nation who suffer from the symptoms and conditions we treat such as: Interstitial Cystitis; Endometriosis; Painful Intercourse; Heavy or Painful Periods; Urinary Frequency or Pain; Vulvodynia or Vulvar Vestibulitis; Pelvic Floor Dysfunction; or Chronic "Female" Pain. These women are seeking a State-of-the-Art diagnostic perspective and the most advanced treatment options available. Many have seen multiple physicians who they feel have not totally grasped the complexity of their problems and have not offered them possible causes of pelvic pain or a way to effectively manage their symptoms. Many have been told there is little or nothing wrong with them -- even that their severe pelvic pain is "all in their heads" -- but these women know better...and they are seeking validation and a better understanding of what is going on with their bodies, so they can feel better and get on with their lives. They want to go back to being the wives, moms, lovers, successful career women -- vital human beings -- they used to be. We are here for these women. Perhaps we can be here for you too.

If you believe the Pelvic Pain Regional Specialty Center might be able to help, we would like to start by learning more about you. Please begin this process by clicking here, so you can download, print, and complete the attached Pelvic Pain Assessment Form. (Please ignore its references to "Provider Comments" and a Chart Number, and note that you do not need to be referred to us by another Provider.) In addition, in lieu of a cover letter and in order to enhance the rather brief "Information About Your Pain" section of the Form, please send us "your story": a brief write-up (no more than 2 type-written pages) consisting of the following - a description of your primary current symptoms, a summary of your pertinent test results, a list of current treatments and treatment history (including impact on your symptoms), an explanation of what (if anything) you believe caused your pain to begin, and a statement of your primary goal or goals (i.e. what you're looking for from us). If you believe it's important that you attach certain key portions of your medical record, that's fine...but please do not send us any films (e.g. x-rays, CT, MRI, etc.). When you have completed the Pelvic Pain Assessment Form and "your story", please mail them to our Center at the address provided below.

The Center's Medical Director will review your information and you also will receive a phone call from a Women's Health Specialist, who will delve further in order to enhance our understanding of your specific situation. At the end of this two-part process, the Pelvic Pain Center staff will do one of three things:

  1. Design a customized individual intensive evaluation and treatment program for you to undergo here in Louisville, KY, based on our initial long-distance assessment of your immediate needs. Most of our out-of-town patients visit with us for 2 days. Please understand that it is difficult to identify conclusively a patient's needs from afar, but we have found that the two-part process outlined above does enable us to do the very best we can at determining whether a trip to our Center might be beneficial to you and anticipating which tests and treatments may be appropriate for you. It also is helpful because it provides our patients with an idea in advance of the out-of-pocket costs involved, since these vary significantly from patient-to-patient and depend upon insurance coverage. We will recommend and outline the specific parameters of your visit here (advise you regarding how many days we believe you might benefit from being here, what tests we recommend, what sort of treatments we foresee, your costs, etc.); or...
  2. Provide you with affirmation that we believe you're already "on the right track" with your current care regimen and do not recommend additional evaluation and treatment at our Center; or...
  3. Suggest that you see another health care professional, who we believe may have a more positive impact on your condition.

The cost of our two-part initial assessment process is $500 (payable by check, Mastercard, or Visa) and payment or credit card information must accompany your information in order for the Medical Director's review to begin. The two-part assessment generally is completed within 2-3 weeks of the arrival of your package at our Center. The medical staff does retain the right to turn down materials for review and, in such a case, will return your payment to you in full. Please keep in mind that sending your information to us and taking part in our assessment does not mean that we have entered into a relationship with you as a patient nor assumed responsibility for any aspect of your care.
